The following questionnaire is only a sample of some fine motor skills that are assessed by an occupational therapist and not a direct indicator that a child needs treatment. This checklist is geared to children ages 5 and above. If your child is below the age of 5 and you have questions please contact Danielle with your concerns.

Does my child have:

¨ Difficulty drawing, coloring, copying, writing, cutting, or avoidance of these activities

¨ Poor pencil grasp

¨ Pencil lines are wobbly, too faint or too dark

¨ Fatigues quickly in paper and pencil tasks

¨ Hand dominance not established

¨ Difficulty with zippers, buttons, or snaps

¨ Appears clumsy or demonstrates difficulty with motor planning
